Services and Fees
NEW PATIENTS
Initial Psychiatric Evaluation (CPT 90792): 90 minutes, $600
ESTABLISHED PATIENTS
Therapy/Medication Management:
55 minutes, $400
Therapy/Medication Management:
25 minutes, $300
Initial Disability Paperwork: $100
Follow-up Disability Paperwork: $50
Prior Authorizations: $25
Letters: $25
Medical Records: $25
Hourly Rate for Legal Issues: $600/hour (including but not limited to depositions, court appearances, review of medical records with summary report for legal cases, or any other legal consultation)
Insurance
Dr. Raphael does not participate in any insurance panels, therefore payment is due at the time of service by cash, check or credit card (Visa, MasterCard, Discover or American Express). A receipt or "superbill" will be provided to you at each visit detailing the services provided, and this superbill may be submitted to your insurance plan for consideration of partial reimbursement for out-of-network benefits.
Please Note: I am NOT a Medicare provider, and my services are not eligible for reimbursement by Medicare. Therefore, you may NOT submit superbills to Medicare for any reimbursement.
Many PPO plans provide some reimbursement for mental health care provided by an out-of-network provider, so you might get a portion of the fees back from your insurance company, depending on your specific plan. Please contact your insurance company to inquire about your out-of-network mental health service benefits if insurance reimbursement is an important issue.
Cancellation and No-Show Policies
A block of time is reserved for your appointment, so if you need to cancel, please give at least 48 business hours of advance notice. Business hours are considered weekdays between Monday 9 am and Friday 5 pm.
Should you cancel within less than 48 business hours of the designated time, or fail to appear for an appointment, you will be charged the full fee for the visit. Your insurance company will likely not reimburse you for missed sessions. Consistent follow-up appointments are important for safe medical care.
